Feedback Forms

Builds the post-service surveys customers fill in. Each form has its questions, a trigger (sent after a completed booking, or issued manually), a response count and a results view.

What this screen is for

A feedback form is a short survey a customer completes after you have done the work. This screen lists the forms you have built, showing each one's name, how many questions it has, what triggers it (after a completed booking, or manually), how many responses it has collected, and whether it is active. From a row you can open the results, edit the form or create a new one.

When to use it

Create a form when you want to collect feedback systematically rather than waiting for people to volunteer it, and come back to read the results. Deleting a form keeps the responses already submitted.

What you see on this screen

The table has these columns:

  • Name — the form's name
  • Trigger — what sends it out
  • Questions — how many it asks
  • Responses — how many have come back
  • Status — whether it is currently in use
  • Actions

Where no forms exist, the page reads "No feedback forms yet" and offers to create the first one.

Filters and actions

There are no filters on this screen. New Form builds one. Each row carries three icon actions: Results, which opens what the form has collected; Edit, which opens the form and its questions; and Delete, which asks you to confirm and states plainly that submitted responses are kept.

Status values

Trigger is one of two values:

  • booking_completed, shown as After completed booking — the form goes out on its own once a booking finishes
  • manual — the form is only ever issued by hand

Status reads Active or Inactive. An inactive form stops being issued but keeps everything it has already collected.
